Diabetes Prevention Programs

Diabetes Prevention Programs (Toronto Central Region)

For individuals 18 years of age or over who may be at risk for developing type 2 diabetes (family, friends, & caregivers may attend).

  • Programs are FREE (OHIP is not required) and offered in many languages
  • TTC fare and childcare may be available upon request.
  • Programs offer the following services at many locations across the region:
    • On-site screening programs at community organizations and events to help people find out if they are at risk of developing type 2 diabetes
    • Healthy living workshops to learn how healthy eating, being active, and managing stress can help to lower the risk of type 2 diabetes.
    • Connection to primary care, individualized diabetes support and other supports when needed (e.g. mental health, housing, or addiction supports)
  • Group programs (offered by registered nurses and/or dietitians) are designed to be have one of the following population focuses:
    • Aboriginal
    • Caribbean
    • East African
    • Latin American
    • South Asian
  • For those whom group programs are not suitable or those who don’t identify with one of the population focuses above diabetes education programs are able to offer support. Visit the Diabetes Education Program page for more information.

  • Groups include: Cooking Groups, Traditional Teaching Circles, Sharing Circles, and Food & Mood Group

  • This program is specially designed to support the South Asian population. 
  • Three part program includes:
    • Part 1: Attend screening
    • Part 2: Atted diabetes prevention workshops
    • Part 3: Attend Chat & Chai (a follow-up group session)
