Facilitating Connections to Diabetes Support

The TDCC Toronto Central Referral Service (TCRS) has facilitated connections to diabetes supports in the community via central intake model for over 10 years. Providing multiple referral method options, people living with and at risk of diabetes are connected to local Diabetes Education Programs (DEPs), Diabetes Eye Screening (DESP), Choose Health Self-Management Program and Diabetes Prevention Programs (DPPs). In alignment with provincial digital priorities, TCRS joined the Ocean eReferral Network in 2023.

As Ontario Health Teams (OHT) develop diabetes integrated care pathways, we have been working with OHT partners to streamline referral pathways to diabetes programs through the TCRS and coordinate outreach efforts to primary care, specialists and people in the community. In 2024/25, we connected with 224 people in the community at a variety of outreach events to share information about available diabetes supports in the community and how to connect with them.

From April 1, 2024 to March 31, 2025 our website had 7,107 users and was visited 10,516 times
